Elevance Survey for Warfighters in Adaptive Sports

Athlete Survey

Move United, with support from Elevance Health, is working to understand what impact adaptive sports has on an athlete’s mental health and happiness. To support this work, we will be surveying Warfighters who are participating in adaptive sports multiple times throughout 2026 and 2027.

Answers to this survey will help Move United understand the significance on adaptive sports in an athlete’s life. The data collected will be important to support future funding for adaptive sports and help with impacting more Warfighters.

Your participation in this survey is entirely voluntary and you may choose not to participate. If you decide to participate in this survey, you may withdraw at any time. Your future participation in adaptive sports programs will not be impacted should you choose not to participate in the survey. Your responses will be anonymous and confidential.

We ask that you please take 5 minutes to answer the questions below as honestly as possible.

Participants who complete both pre- and post- surveys will receive a giveaway from Move United.
