2026 The Hartford Nationals
Swimming Relay Form

This form is due no later than 7:00pm on Monday, July 13th

This form is to be completed by the Head Coach from each team.

Refer to the Relay Points matrix below. Each relay team must consist of at minimum 3 athletes from the same team (team may only substitute 1 member from outside the team). Each athlete on the relay team must have a qualifying standard in the distance being swum (25 distance, swimmer may have a 50 time divided by 2 for a qualifying time). One substitute is allowed.

Each athlete may enter no more than 2 relays.

Please put names in race order.

2026 Changes

Elimination of all male and all female relays. Only mixed gender relays will be contested. Mixed gender relays may consist of male and female swimmers, all male swimmers, or all female swimmers.
Elimination of 10 point 100 yard freestyle relay and 15 point 100 meter freestyle relay.
