Each year prior to the start of the season, Team Reviewers must submit their team rosters and Team Sport Proposed Classifications for any player who does not already have a Proposed or Confirmed class. Please use the form below to upload your team’s roster and any information to support your classification proposal.

This form will allow you to upload information for up to 14 players at one time. For each player, EVERY field must be filled in to qualify for a Sport Class Team Proposed Classification.
2025 Classification Deadlines:
- Team Classifier Registration Completed – June 15
- Athlete Proposed Class & Screening Forms Due – July 24
No late submissions will be allowed without written approval from Move United.
