Walter Reed National Military Medical Center Warfighter Referral Form

Once you complete this form, eligible athletes will be approved for the Move United Warfighters Program. If you don’t meet the eligibility criteria, you’ll still be added to our general information list to stay connected with Move United opportunities.

Athletes must meet the following criteria to qualify for the Warfighters Program:

  • Must have permanent physical disability
  • Disabilities served include (but are not limited to) amputation, traumatic brain injury, visual impairments, muscle, burn and nerve damage and spinal cord injury.
  • Includes any veteran or service member injured Pre or Post 2001, combat or non-combat injured
