World Para Dance Sport Athlete License Request Form
Please complete the form below to request your 2025 World Para Dance Sport Athlete License. Your request will not be submitted to World Para Dance Sport until the below form is completed and your $25 fee is paid via the Move United payment portal.
If you were not licensed in 2024, you also must become a member of Move United in order to secure your license. You can sign up, for free, at https://moveunitedsport.org/membership-level/individual.
A 2025 license is valid until December 31, 2025.
