World Para Dance Sport Athlete License Request Form
Please complete the form below to request your 2026 World Para Dance Sport Athlete License. Your request will not be submitted to World Para Dance Sport until the below form is completed and your $25 fee is paid via the Move United payment portal.
In order to receive your license, you must also become a member of Move United. You can sign up or renew your membership at https://moveunitedsport.org/membership-level/individual. Please note there are two membership options. One is free and one has a fee. A free membership is all that is required of you for licensing.
A 2026 license is valid until December 31, 2026.
