Member Organization Update Form

  • If you have not recently, please provide updated contact information for your organization. We want to ensure that all member related communications are getting to the right contacts, and that our public listings of your member organization are accurate.
  • Member Organization Contacts

    Please provide updated contact information for your organizations's Primary and Secondary Contacts. Descriptions of the nature of these designations can be found with each entry, but both Primary & Secondary contacts will be added to distribution list for any member related newsletters and announcements (i.e. grant and training opportunities, etc.).
  • This representative will be the contact with which Move United will communicate on higher level areas (i.e. insurance, compliance, etc.). This individual is typically the Executive Director or Board President.
