You have been invited to apply for the Move United PING Club Donation Program. Please note the following:
-All attendees who complete at least 6 of 8 sessions of the Links to Freedom (L2F) Golf Clinic Series will be considered eligible to apply, but must meet the Move United eligibility criteria and fully complete this application for consideration.

Links to Freedom (L2F)
For L2F eligibility consideration, all students must complete a minimum of 6 of the 8 total golf clinic sessions to be considered eligible to apply for the Move United Ping Club Donation Program. It is each student’s sole responsibility to sign in every session to be credited with attendance for that day.
Move United
Move United’s mission is to provide national leadership and opportunities for individuals with disabilities to develop independence, confidence, and fitness through participation in community sports, recreation and educational programs.
To qualify for Move United Warfighters you must be a military member (pre and post 9/11) who has sustained a permanent physical disability (combat or non-combat injured) and need adaptive instruction or equipment. Disabilities served include (but are not limited to) amputation, TBI, visual impairments, burn damage, severe muscle and nerve damage and spinal cord injury.
Eligible wounded veterans and service members who attend a minimum of six clinics in a single series may qualify for a custom fitted set of PING clubs. It is the service member’s responsibility to sign in each week to have attendance counted towards clubs.
Club distribution is at the sole discretion of L2F and Move United based on program qualifications and club availability.
You will need to sign waivers for Move United. All waivers can be completed online.
