This is the reporting form for the Monthly Virtual Matches (Orion Scoring System), ran through the Move United Adaptive Shooting League.

It is recommended that this template form is fully completed and ready to attach before you begin to submit this report.

Uploaded files may not exceed 10MB (combined). This is the most common reason for forms not coming through. Additional files can be e-mailed to mvenus@moveunitedsport.org

As an incentive, each month a reporting form is submitted, your organization will be entered into a drawing for an Orion Membership Grant, provided by Move United. This grant, valued up to $500, will be for the reimbursement of 1 or 2 year’s worth of an Orion Membership (dependent on first year vs. renewal membership).
